Factors Affection the Negative Exploratory Lpaprotomy and Negative Appendectomy in Acute Appendicitis

Abstract
Acute appendicitis is one of the most common perplexing diagnostic problems confronting physicians, after present with atypical findings that make accurate preoperative diagnosis difficult. Problem related to the diagnosis of appendicitis are
evidenced
by the significant negative laparotomy rate.
This review is a clinical review of 271 cases of acute appendicitis, which were operated on Department of Surgery, Chuncheon Sacred Heart Hospital, Hallym University from January, 1991 to December, 1993.. Each case was divided one of the two
groups
according to whether the pathologic diagnosis was appendicitis or not appendicitis and analyzed to the clinical presentation of the physical findings and the results of laboratory studies including leukocyte count and urinalysis. These groups
were
compared by the method of comparison of means, student t-test. The purpose of this study was to identify and evaluate those portion that are most helpful in making the correct diagnosis of acute appendicitis.
As a result, following factors were proved to have differentiation and statistical significance(p¡Â0.05) and these were; 1) duration of symptom, 2) The nature of pain, 3) involuntary abdominal rigidity, 4) decreased bowel sound, 5) RLQ tenderness
on
rectal examination, 6) elevation of body temperature, 7) Leukocyte count. The positive appendectomy rate was 90.4%. The negative appendectomy rate and negative laparotomy rate were 9.4% and 8.5%.
We conclud that there was a significant statistical differences between the two groups, and these were potentially good parameters of acute appendicitis.
